Looking at the EIP AHA website, part of its function, to quote from its own text is to act as as innovation marketplace and "a communication and information hub for all actors involved in Active and Healthy Ageing throughout Europe". As a startup, this is one of the mutual benefits that we want to achieve by contributing and learning from experienced professionals in the area of older care. In short we regard the EIP AHA as a channel for problem and solution validation.
The Integrated Care working group is focused on using integrated care methodologies involving health & social care to reduce unnecessary hospitalisations and improve the quality of chronic care. As with our objectives in Assesspatients, its about enabling a better collaboration between the older person and the various care disciplines in the assessment, management and delivery of care.
To illustrate where Assesspatients plan to contribute to the EIP AHA initiative, I have summarised our planned contributions to the three main objectives of the Partnership.
Healthy Life Years
Best practice in older care recommends a multidisciplinary approach involving healthcare and social services and a central focus on the older person.
These two characteristics are a central part of our web based solution that enables better collaboration between the older person and their multidisciplinary care team. Our goal is to enable better quality of care which helps support a better healthy and active life for the older person.
Sustainable care systems
In our solution we generate a series of metric driven dashboards collated from individual older people care updates. The metrics relate to person’s care experience, quality of care, care team effectiveness and financial measurements. These dashboards enable the care programme management to manage the delivery of better care in a cost effective manner.
As an ehealth startup, we have the ability to scale and grow based on the achievement of agreed targets and delivery of benefits related to the quality of life for older people. This growth will provide additional employment and the opportunity to partner with other companies and academia to research and deliver innovation in older care.
